FDA Approves Orforglipron: The New Oral GLP-1 Pill for Weight Loss

More than 100 million adults in the United States are living with obesity, yet fewer than 1 in 10 people who could benefit from a GLP-1 medication are currently taking one. For many, the barrier is not willingness. It is access, needle aversion, or the complexity of building a new treatment routine into daily life.

That changed on April 1, 2026, when the U.S. Food and Drug Administration approved Foundayo™ (orforglipron), the first oral GLP-1 receptor agonist for chronic weight management that can be taken at any time of day, with no restrictions around food or water.

For the team at Health Research of Hampton Roads, the announcement carries particular meaning. HRHR worked alongside Eli Lilly over several years to help advance the research that led to this approval, and that contribution reflects what clinical participation in this community makes possible.

What Is Orforglipron?

Orforglipron is a once-daily oral medication in the GLP-1 receptor agonist class, the same class as several well-known injectable treatments for obesity and type 2 diabetes. What distinguishes it is its form and flexibility. It is a small-molecule, non-peptide pill that requires no injections, no cold storage, and no planning around mealtimes.

GLP-1, or glucagon-like peptide-1, is a hormone the body naturally produces after eating. It helps regulate appetite, blood sugar, and digestion. GLP-1 receptor agonists work by activating the same pathways, reducing hunger signals and supporting more stable metabolic function. Until this approval, every FDA-approved GLP-1 treatment for weight management required a subcutaneous injection.

Orforglipron was originally discovered by Chugai Pharmaceutical and licensed by Eli Lilly in 2018. In addition to weight management, it is currently being studied as a potential treatment for type 2 diabetes, obstructive sleep apnea, hypertension, and other metabolic conditions.

What the Clinical Trial Results Showed

The FDA approval was supported by the ATTAIN Phase 3 clinical trial program, which enrolled more than 4,500 adults with obesity or overweight across two global studies.

In ATTAIN-1, a 72-week randomized, double-blind, placebo-controlled trial, participants on the highest dose of orforglipron who remained on treatment lost an average of 27.3 pounds, representing a 12.4 percent reduction in body weight. The placebo group lost an average of 2.2 pounds over the same period. Across all participants, regardless of study completion, the average weight loss was 25 pounds compared to 5.3 pounds with placebo.

Beyond weight, the ATTAIN program showed meaningful reductions in several cardiovascular risk markers, including waist circumference, non-HDL cholesterol, triglycerides, and systolic blood pressure. The complete trial record is registered at ClinicalTrials.gov under identifier NCT05869903.

Why an Oral Weight Loss Pill Changes the Conversation

Obesity is a chronic, complex condition shaped by genetics, metabolism, environment, and behavior. According to the National Institute of Diabetes and Digestive and Kidney Diseases, more than 2 in 5 U.S. adults have obesity, a condition that significantly raises the risk of heart disease, type 2 diabetes, certain cancers, and other serious health complications.

Despite this, access to effective treatment has remained uneven. The CDC reports that adult obesity has continued to rise even as treatment options have expanded, with barriers ranging from cost and stigma to the practical realities of injection-based therapies.

An oral GLP-1 pill removes one of the most common of those barriers. Because orforglipron has no food or water restrictions, it fits more naturally into varied daily routines. Eli Lilly has also taken steps to improve access to the medication, with programs aimed at expanding affordability and availability for eligible patients.

Clinical research does not happen in a vacuum. It depends on the volunteers who show up, follow study protocols, and commit months or years of their time to advancing a treatment that may one day help millions of people they will never meet.

Health Research of Hampton Roads had the privilege of working with Eli Lilly throughout the orforglipron development program, contributing to the data and patient experience that helped bring this treatment to approval. Since 1998, HRHR has served as an FDA-regulated clinical research site in Newport News, and since 2017, the site has supported the development of 10 treatments that have reached the market.

What Comes Next

Orforglipron’s approval is a milestone, but research into this class of medications is far from finished. Lilly has submitted the drug for regulatory review in more than 40 countries, and ongoing studies continue to explore its applications in type 2 diabetes, sleep apnea, hypertension, and other conditions.

This trial is for obese non-diabetic adults with knee osteoarthritis, and who are highly motivated to lose weight.

Eligibility

  • Must NOT have diabetes
  • Must NOT have had a previous weight loss surgery
  • MUST be available for morning office visits only. No afternoon office visits are available
  • No weight loss medication use in the last 12 months
  • MUST be willing to have a knee X-ray performed
